Rotator Cuff Tendinopathy & Impingement
Also called: Shoulder tendinitis, subacromial pain
Irritation of the cuff tendons that steer the shoulder's ball in its shallow socket — the most common cause of shoulder pain. Driven by load spikes, overhead volume, posture, and how the shoulder blade moves. Responds impressively to progressive exercise; most cases never need anything more invasive.
Common signs
- Outer-shoulder/upper-arm ache, worse reaching overhead or behind the back
- A painful arc raising the arm to the side
- Night pain lying on that shoulder
- Weak or painful rotation strength testing
Seek help now if
- Sudden weakness after trauma (acute tear — assess promptly)
- Hot swollen joint with fever
- Rapidly progressive weakness or wasting
Any of these: call 911 or get to an emergency room first. Everything else can wait.
What to do first
- Get examined — cuff, biceps, AC joint, neck referral, and frozen shoulder present similarly and are treated differently
- Modify (don't eliminate) overhead volume; keep the shoulder moving through comfortable ranges
- Begin cuff and scapular loading — exercise is the first-line, best-evidenced treatment
If it's become chronic
- A 12-week progressive program (cuff rotation work, scapular control, posture, thoracic mobility) is the standard of care
- Refractory cases: re-examine the diagnosis, training loads, and neck contribution before escalating
- Injections can quiet a hot shoulder for rehab to proceed — they treat symptoms, exercise treats causes
