Sciatica / Lumbar Radiculopathy
Also called: Pinched nerve in the low back
Irritation of a lumbar or sacral nerve root producing pain, tingling, or weakness along its path down the leg. 'Sciatica' describes the symptom; the cause is usually a disc herniation or a narrowed nerve canal — and identifying which changes the treatment.
Common signs
- Sharp, burning, or electric pain from the buttock down the leg, often below the knee
- Numbness or pins-and-needles in a specific strip of leg or foot
- Weakness: toe-walking or heel-walking difficulty
- Worse with sitting, bending, cough/sneeze (disc pattern) or with standing/walking, eased by sitting (stenosis pattern)
Seek help now if
- Saddle numbness or bladder/bowel changes — emergency
- Progressive weakness or foot drop
- Both legs involved
- Night pain with systemic symptoms
Any of these: call 911 or get to an emergency room first. Everything else can wait.
What to do first
- Precise exam identifies the level and the cause-pattern — treatment differs between disc-driven and stenosis-driven sciatica
- Track the territory of symptoms weekly; shrinking, centralizing symptoms = healing
- Stay as active as symptoms allow; complete rest slows recovery
If it's become chronic
- Beyond 6–12 weeks: MRI correlation, layered conservative care, possible co-management (e.g., epidural injection as a bridge) while rehab continues
- Address the whole chain: hip mobility, nerve mobility, core endurance, gait
- Most cases resolve without surgery; deficits that progress change that calculus
