Cervicogenic Headache
Also called: Headache from the neck
A headache generated by structures in the upper neck — joints, discs, muscles, nerves — and referred to the head. Usually one-sided, starting at the base of the skull and wrapping forward. Frequently misdiagnosed as migraine or tension headache, which is why neck-focused treatment often succeeds where medication has failed.
Common signs
- One-sided headache starting at the skull base, spreading toward the temple or eye
- Triggered or worsened by neck movement or sustained postures
- Reduced neck range of motion on the affected side
- Tenderness in the upper neck joints and suboccipital muscles
Seek help now if
- Thunderclap ('worst ever') sudden headache — emergency
- New headache with fever, stiff neck, rash, or confusion
- New headache pattern after age 50
- Headache with neurological deficits (vision loss, weakness, speech changes)
Any of these: call 911 or get to an emergency room first. Everything else can wait.
What to do first
- An upper cervical examination can usually reproduce the headache from the neck — that reproduction confirms the source
- Track triggers: desk time, driving, sleep position
- Address the neck first; these headaches typically don't respond well to headache medication alone
If it's become chronic
