Complex Regional Pain Syndrome (CRPS)
Also called: RSD, causalgia
A limb pain condition where the nervous system's protection response misfires after an injury (often minor — a sprain, fracture, or surgery), producing pain wildly out of proportion with skin, temperature, and swelling changes. The single most important fact: early recognition and active movement-based treatment dramatically improve outcomes — this diagnosis punishes delay and immobilization.
Common signs
- Burning, severe pain in a limb exceeding the original injury's explanation
- Skin changes: color shifts (red/purple/mottled), temperature differences, unusual sweating
- Swelling and profound touch sensitivity (light touch feels like fire)
- Stiffness, tremor, or weakness; later, skin/nail/hair changes
- Guarding and fear of using the limb
Seek help now if
- This condition is its own urgency: pain + color/temperature/swelling changes after injury deserves evaluation within days, not months
- Signs of infection (fever, spreading redness) — different emergency
- Rapidly progressive weakness
Any of these: call 911 or get to an emergency room first. Everything else can wait.
What to do first
- Seek evaluation quickly if the pattern fits — the treatment window matters more here than almost anywhere in pain medicine
- Begin gentle, graded use of the limb immediately as guided — motion is medicine and disuse is fuel
- Start desensitization: progressive texture exposure retrains touch processing
If it's become chronic
- The active model wins: graded motor imagery, mirror therapy, desensitization, progressive functional loading — a nervous-system retraining project with real evidence
- Multidisciplinary care is appropriate and often necessary — pain specialists, movement care, and nervous-system regulation working together
