Polymyalgia Rheumatica
Also called: PMR
An inflammatory condition of adults over 50 causing profound aching and morning stiffness across both shoulders and hips — often arriving fast, sometimes 'overnight,' making dressing and rising from chairs suddenly difficult. Common signs

- Symptoms with prominent joint swelling of hands/feet (reconsider RA)
- Fever/weight loss dominating the picture (broader workup)
Any of these: call 911 or get to an emergency room first. Everything else can wait.
What to do first
- Get labs promptly: inflammatory markers (ESR/CRP) are typically markedly elevated — this diagnosis moves fast once suspected
- Screen out the cousin explicitly — headache/jaw/vision questions are part of every PMR conversation
